dc.descriptionAntecedentes: El reflujo vesico-ureteral (RVU) es una de patología importante en pediatría, diagnosticada generalmente luego de un episodio de infección urinaria. Su incidencia parece haber aumentado como consecuencia del uso de la ecografía que ha llevado a una mayor detección en la etapa antenatal y postnatal. El reflujo vesico ureteral trae consigo riesgo de cicatrices renales, aunque aún es muy controversial. En los niños con primer episodio de ITU, la incidencia de RVU va ría entre 20% y 40%(2) Objetivo: Establecer la asociación entre grado de reflujo vesico-ureteral primario y cicatrices renales en pacientes pediátricos con infección urinaria confirmada. Materiales y Métodos: se efectuó un estudio cuantitativo, observacional analítico, retrospectivo en el que se incluyó de manera aleatoria una muestra de 49 pacientes de entre 0 a 17 años con RVU e infección urinaria confirmada, con estudio de gammagrafía con DMSA para establecer la presencia de cicatriz renal, los cuales cumplieron con los criterios de selección y que fueron atendidos en el hospital Roberto Gilbert en Guayaquil entre el 1 de enero de 2014 al 31 de diciembre de 2015. Resultados: La edad promedio fue de 4 ± 3 años. El índice mujer:hombre fue de 1,45 a 1. En el 73,5% de los casos, el agente causal fue la E. coli, el 22,4%; de estos era BLEE y el 2% multirresistente. El 95,5% de los pacientes tenía profilaxis antibiótica. La mayor parte de los casos de RVU eran de grado IV (30,6%). Se presento una mayor proporción de pacientes con cicatriz renal en el grado III de reflujo (71,4%), tendencia sin significado estadístico (P 0,778) Conclusiones: En los niños de atendidos en este hospital no se logró verificar asociación entre el grado de RVU y el desarrollo de cicatrices renales; es posible que la causa sea de origen multifactorial y que influya una predisposición genética. Se requieren más estudios al respecto.en_US
dc.description.abstractBackground: Vesico-ureteral reflux (VUR) is one of important pathology in pediatrics, usually diagnosed after an episode of urinary infection. Its incidence seems to have increased as a result of the use of ultrasound that has led to greater detection in the antenata l and postnatal stages. Ureteralvesical reflux brings with it the risk of renal scars, although it is still very controversial. In children with first episode of UTI, the incidence of VUR ranges between 20% and 40% (2) Objective: To establish the association between the grade of primary vesico-ureteral reflux and renal scars in pediatric patients with confirmed urinary infection. Materials and Methods: A quantitative, analytical, retrospective observational study was conducted in which a sample of 49 patients aged 0 to 1 7 years with VUR and urinary infection confirmed with a DMSA scintigraphy study was randomly included to establish the presence of renal scar, which met the selection criteria and which were treated at the Roberto Gilbert hospital in Guayaquil between January 1, 2014 and December 31, 2015. Results: The average age was 4 ± 3 years. The female: male index was 1.45 to 1. In 73.5% of the cases, the causative agent was E. coli, 22.4%; of these was ESBL and 2% multi-resistant. 95.5% of the patients had antibiotic prophylaxis. The majority of VUR cases were grade IV (30.6%). There was a higher proportion of patients with renal scar in grade III reflux (71.4%), a trend without statistical significance (P 0.778). Conclusions: In children treated at this hospital, no association was found between the degree of VUR and the development of renal scars; it is possible that the cause is of multifactorial origin and that it influences a genetic predisposition. More studies are required in this regard.en_US
