Análisis comparativo de la efectividad de diferentes técnicas de irrigación en modelo 3D de istmos radiculares.

Abstract

Introduction: The isthmus is a narrow canal that connects two root canals and can accumulate pulp remnants, dead tissue, and microorganisms, increasing the risk of reinfection. Despite advancements in irrigation and disinfection techniques, areas such as the isthmus and lateral canals remain challenging to treat with conventional methods. Objective: To compare the effectiveness of different irrigation techniques in 3D models of root canal isthmuses. Materials and Methods: Thirty 3D printed root canal models were created, each with two parallel canals with a 25° curvature and dimensions of 25/06, with a total length of 15 mm. The canals were connected by an isthmus 2.5 mm long, 0.4 mm wide, and 0.2 mm high, located 7 mm from the apex. A 5.25% NaCOl solution was used as the irrigant. Three groups were formed (1: manual irrigation, 2: EDDY tips, and 3: Endo Activator), each consisting of 10 models. Organic tissue was simulated with canned meat to fill the canals. Results: Significant differences were observed among the three techniques in the isthmus, with the EDDY technique achieving the highest removal of organic tissue. In the coronal and middle thirds, all techniques proved to be highly effective, while in the apical third, the EndoActivator and EDDY techniques showed superior results compared to the manual technique. Conclusions: The use of irrigation with the EDDY technique and 5.25% NaOCl is recommended in the isthmus. In the middle and apical thirds, sonic activation techniques are advised.
