| dc.description | La lumbalgia crónica constituye un importante problema de salud ocupacional en el personal militar debido a las elevadas exigencias físicas propias de sus actividades, entre ellas el transporte frecuente de equipo táctico, armamento y elementos de protección. El objetivo de esta investigación fue determinar la prevalencia de lumbalgia crónica y su asociación con la carga del equipo táctico en militares de 22 a 45 años atendidos en el Hospital Naval de Guayaquil durante el período 2022-2025. Se realizó un estudio observacional, analítico, retrospectivo y de corte transversal, con enfoque cuantitativo, mediante la revisión de registros clínicos. Se analizaron 584 pacientes militares, considerando variables sociodemográficas y ocupacionales como edad, sexo, índice de masa corporal, tiempo de servicio, peso del equipo táctico y frecuencia de entrenamiento físico. La prevalencia global de lumbalgia crónica fue del 59,6 % (IC 95 %: 55,6-63,6), con la mayor prevalencia registrada en 2023 (64,1 %). Los militares con lumbalgia crónica transportaban un peso promedio de equipo táctico significativamente mayor que aquellos sin la patología (28,7 kg frente a 23,1 kg; p < 0,001). El análisis de regresión logística evidenció que, por cada kilogramo adicional de equipo transportado, las probabilidades de presentar lumbalgia crónica fueron 18 % mayores (OR = 1,18; IC 95 %: 1,12-1,24; p < 0,001), ajustando por las variables incluidas en el modelo. Asimismo, la mayor edad y el incremento del índice de masa corporal mostraron asociación significativa con la presencia de lumbalgia. Entre las principales limitaciones funcionales se identificaron la dificultad para realizar marchas prolongadas (61,5 %), la limitación para levantar objetos pesados (53,7 %) y la reducción de la velocidad de desplazamiento (38,5 %), mientras que el ausentismo laboral alcanzó el 25,6 %. Se concluye que la lumbalgia crónica presenta una elevada prevalencia en la población militar estudiada y existe una asociación estadísticamente significativa entre una mayor carga de equipo táctico y la presencia de esta patología. Estos resultados resaltan la importancia de fortalecer las estrategias de prevención, vigilancia de la salud ocupacional y optimización ergonómica del equipamiento militar. | en_US |
| dc.description.abstract | Chronic low back pain represents an important occupational health problem among military personnel due to the high physical demands associated with their activities, including the frequent transportation of tactical equipment, weapons, and protective gear. The objective of this study was to determine the prevalence of chronic low back pain and its association with tactical equipment load among military personnel aged 22 to 45 years treated at the Naval Hospital of Guayaquil between 2022 and 2025. An observational, analytical, retrospective, cross-sectional study with a quantitative approach was conducted through the review of clinical records. A total of 584 military patients were analyzed, considering sociodemographic and occupational variables such as age, sex, body mass index, years of military service, tactical equipment weight, and frequency of physical training. The overall prevalence of chronic low back pain was 59.6% (95% CI: 55.6-63.6), with the highest prevalence recorded in 2023 (64.1%). Military personnel with chronic low back pain carried a significantly greater average tactical equipment load than those without the condition (28.7 kg versus 23.1 kg; p < 0.001). Logistic regression analysis showed that, for each additional kilogram of equipment carried, the odds of chronic low back pain were 18% higher (OR = 1.18; 95% CI: 1.12-1.24; p < 0.001), adjusting for the variables included in the model. Older age and higher body mass index were also significantly associated with the presence of chronic low back pain. The main functional limitations identified were difficulty performing prolonged marches (61.5%), limitations in lifting heavy objects (53.7%), and reduced movement speed (38.5%), while work absenteeism reached 25.6%. It was concluded that chronic low back pain has a high prevalence among the military population studied and that there is a statistically significant association between greater tactical equipment load and the presence of this condition.
