Titre: Prevalencia de infección de vía urirnaria recurrente en pacientes con profiliaxis antibiótica por reflujo vesicoureteral atendidos en el servicio de pediatría del Hospital Roberto Gilbert durante el periodo 2015 - 2017.

Résumé: Background:Recurrent urinary tract infection in patients withvesico-ureteral reflux is a frequent condition, leading to complications. Management depends on the pathogen and the resistance to antibiotics that it presents. Materials and Methods:Cross-sectional Analytical Observational Study, carried out in 96 patients with vesico-ureteral reflux treated in the Pediatric Service of the Hospital Roberto Gilbert Elizalde during the period2015-2017, with the objective of determining the prevalence of recurrent UTI in this group. Results:A prevalence of recurrent UTI was obtained in patients with vesicoureteral reflux of 35.42%. The recurrent urinary tract infection is 1.06 times more frequent in females (p = 0.83, OR = 0.94, CI: 0.59-1.62), it is 1.14 times more frequent in females. patients of rural origin, the risk of presenting a beta-lactamase-producing bacterium in patients with recurrent UTI is 2.04 times more compared to patients who do not have recurrent UTI (OR = 0.49, CI: 0.29-0 , 86), 95.84% of patients were sensitiveto the antibiotic used in prophylaxis (p = 0.000), the risk of presenting recurrent UTI of patients who present a complication in the urinary system is 1.72 times more, compared to patients who do not have recurrent UTI (OR = 0.58, CI: 0.34-0.99). Conclusions:There is a high prevalence of UTI in patients withvesico-ureteral reflux. The prophylactic antibiotic management offered is in accordance with the sensitivity of the microorganism in most cases. The high number of complications is related to the recurrence of the UTI.
Description: Antecedentes: La infección de vías urinarias (IVU) recurrente en los pacientes con reflujo vésico-ureteral es una condición frecuente, que acarrea complicaciones. El manejo depende del patógeno y dela resistencia a los antibióticos que éste presente. Materiales y Métodos: Estudio Observacional Analítico de Corte Transversal, realizado en 96 pacientes con reflujo vésicoureteral atendidos en el Servicio de Pediatría del Hospital Roberto Gilbert Elizalde durante el periodo 2015-2017, con el objetivo de determinar la prevalencia de IVU recurrente en este grupo. Resultados:Se obtuvo una prevalencia de IVU recurrente en los pacientes con reflujo vésicoureteral del 35,42%. La infección de vías urinarias recurrente se presenta 1,06 veces más en el sexo femenino (p=0,83; OR=0,94; IC: 0,59-1,62), es 1,14 veces más frecuente en los pacientes de procedencia rural, el riesgo de presentar una bacteria productora de betalacta masa en los pacientes con IVU recurrente es 2,04 veces más en comparación con los pacientes que no presentan reflujo vésicoureteral (OR=0,49; IC: 0,29-0,86), el 95,84% de los pacientes eran sensibles al antibiótico utilizado en la profilaxis (p=0,000), el riesgo de presentar IVU recurrente de los pacientes que presentan una complicación en el sistema urinario es 1,72 veces más, en comparación con los pacientes que no presentan complicación del sistema urinario (OR=0,58; IC: 0,34-0,99). Conclusiones:Existe una elevada prevalencia de IVU en pacientes con reflujovésico-ureteral. El manejo antibiótico profiláctico ofrecido se encuentra acorde a la sensibilidad del microorganismo en la mayoría de los casos. El elevado número de complicaciones se relaciona con la recurrencia de la IVU.
