Título : Identificación de infecciones nosocomiales en pacientes con VIH/SIDA en UCI en el Hospital General del Norte de Guayaquil 2017 - 2019.

Resumen : Introduction: Nosocomial infections are classified as: An infection that is contracted within the hospital by a patient who has been hospitalized for a cause other than said infection. Or an infection that appears in a patient admitted to a hospital or some other health care center where the infection had not presented symptoms or was in the incubation period until the moment of being admitted. Objectives: To identify the most frequent nosocomial infections that occur in HIV / AIDS patients in the ICU at the General Hospital Del Norte de Guayaquil Los Ceibos 2017 - 2019 Materials and methods: A retrospective cohort study. The quantity of the sample was conditioned to the total universe of HIV / AIDS patients with hospital stay in the ICU service of the Hospital del Norte de Guayaquil Ceibos. The sample (84 patients) was selected, the subjects who were diagnosed with HIV / AIDS and presented opportunistic diseases during their hospital stay were considered. Results: 88.2% of the population consisted of men, with a mean age of 41.8 years. The most frequent isolated microorganism for nosocomial bacteremia was Pseudomona aeruginosa in 38% of the cases, and the most frequent pathology in relation to nosocomial disease was pneumonia with 36.9% of the cases presented. Conclusions: A comparative analysis of means was performed, which did not demonstrate the statistically significant difference estimated, which shows a probable lack of relationship between HIV infection of our patients with the development of nosocomial infections, but could be established by factors external, be these comorbidities, or clinical status in which they were presented at the admission of ICU, and hospital stay times, not detailed in this study Conclusions: A comparative analysis was performed, which did not show the statistically significant difference estimated, which shows a probable lack of relationship between HIV infection of our patients with the development of nosocomial infections, but could be established by external factors, be these comorbidities, or clinical status in which they were presented at the admission of ICU, and hospital stay times, not detailed in this study.
Descripción : Introducción: Las infecciones nosocomiales son catalogadas como: Una infección que se contrae dentro del ámbito hospitalario por un paciente que ha sido internado por una causa distinta a dicha infección. O una infección que aparece en un paciente internado en un hospital o algún otro centro de atención de salud en quien la infección no había presentado sintomatología ni estaba en periodo de incubación hasta el momento de ser internado. Objetivos: Identificar las infecciones nosocomiales más frecuentes que se presentan en pacientes VIH/SIDA en UCI en el Hospital General del Norte de Guayaquil Los Ceibos 2017 - 2019 Materiales y métodos: Se realizó un estudio de cohorte retrospectivo. La cantidad de la muestra estuvo condicionado al universo total de pacientes VIH/sida con estancia hospitalaria en el servicio UCI del Hospital del Norte de Guayaquil Ceibos. La muestra (84 pacientes) se seleccionó, se tuvo en cuenta los sujetos que fueron diagnosticados con VIH/Sida y presentaron enfermedades oportunistas durante su estancia hospitalaria.Resultados: El 88.2% de la población estuvo constituido por hombres, con una media de edad de 41.8 años. La población del estudio obtuvo cargas virales indetectables en el 77.6% de los casos. El microorganismo aislado más frecuente para la bacteriemia nosocomial presentada por lo mismos fue de Pseudomona aeruginosa en el 38% de los casos (Gráfico 1), y la patología más frecuente en relación a enfermedad nosocomial fue neumonía con el 36,9% de los casos presentados. Conclusiones: Se realizó un análisis comparativo de medias, el cual no demostró la diferencia estadísticamente significativa estimada, lo que muestra una probable falta de relación entre la infección por VIH de nuestros pacientes con el desarrollo de infecciones nosocomiales, sino que pudiese estar establecida por factores externos, sean estas comorbilidades, o estado clínico en el que se presentaron los mismos al ingreso de UCI, y tiempos de estadía hospitalaria, no detallados en este estudio.
