Título : Prevalencia de infecciones en pacientes con artritis reumatoide bajo tratamiento con modificadores de la respuesta biológica en el Hospital IESS Teodoro Maldonado Carbo del 2017 al 2019.

Resumen : Introduction: Rheumatoid Arthritis is a systemic autoimmune disease of unknown origin that affects the joints bilaterally and symmetrically. One of the therapeutic options is the biological response modifiers when the primary treatment objective is not achieved, through the use of conventional drugs. However, one of the most frequent complications of this disease with the use of these drugs is infections. Objective: Identify the prevalence of infections in patients diagnosed with Rheumatoid Arthritis under treatment with biological response modifiers at the IESS Teodoro Maldonado Carbo Hospital from 2017 to 2019. Methods: An observational, descriptive, cross-sectional study was carried out in patients from the IESS Teodoro Hospital Maldonado Carbo from 2017 to 2019, with a diagnosis of Rheumatoid Arthritis as established in the ACR / EULAR 2010 classification criteria, analyzing patients who were under treatment with biological agents. Results: Among the 162 patients, the presence of infections was detected in 108 patients (66.7%). The type of infection with the highest prevalence was genitourinary, which occurred in 48 (44.44%) of the patients. The drug most associated with infections was Etanercept. 50% of the patients had comorbidities. Conclusions: The prevalence of infections in patients with Rheumatoid Arthritis who are receiving treatment with biological response modifiers was 66.7%. The most common type of infection among these patients was urogenital, followed by respiratory infections. Half of the patients lacked comorbidities, so it cannot be affirmed that the presence of these, conditions a higher risk of the infections in the patient population.
Descripción : Introducción: La Artritis Reumatoide es una enfermedad sistémica autoinmune de origen desconocido que afecta a las articulaciones de forma bilateral y simétrica. Una de las opciones terapéuticas son los modificadores de la respuesta biológica cuando no se logra alcanzar el objetivo de tratamiento primario mediante el uso de fármacos convencionales. Sin embargo, una de las complicaciones más frecuentes con el uso de estos fármacos son las infecciones. Objetivo: Identificar la prevalencia de infecciones en pacientes diagnosticados con Artritis Reumatoide bajo tratamiento con modificadores de la respuesta biológica en el Hospital IESS Teodoro Maldonado Carbo del 2017 al 2019. Métodos: Se realizó un estudio observacional, descriptivo, de corte transversal en pacientes del Hospital IESS Teodoro Maldonado Carbo del 2017 al 2019, con diagnóstico de Artritis Reumatoide según lo establecido en los criterios clasificatorios ACR/EULAR 2010, analizando a los pacientes que estaban bajo tratamiento con fármacos biológicos. Resultados: Entre los 162 pacientes, la presencia de infecciones se detectó en 108 pacientes (66.7%). El tipo de infección con mayor prevalencia fue el genitourinario, que se presentó en 48 (44.44%) de los pacientes. El fármaco que más se relacionó con infecciones fue el Etanercept. El 50% de los pacientes tenía comorbilidades. Conclusiones: La prevalencia de infecciones en pacientes con Artritis Reumatoide que se encuentran recibiendo tratamiento con modificadores de la respuesta biológica fue del 66.7%. El tipo de infección más frecuente entre estos pacientes fue el urogenital, seguidas de las de tipo respiratorio. La mitad de los pacientes carecieron de comorbilidades por lo que no se puede afirmar que la presencia de éstas, condiciona mayor riesgo de infecciones en la población de pacientes.
