Título : Valor pronóstico del péptido natriurético n-terminal-pro-cerebral (nt-probnp) en la mortalidad intrahospitalaria de los pacientes con insuficiencia cardiaca Hospital Teodoro Maldonado Carbo durante el periodo 2020 – 2021.

Resumen : Introduction: N-terminal pro-brain natriuretic peptide (NT-proBNP) has been extensively studied for its predictive ability in patients with heart failure. Currently, NT-proBNP has been shown to be clinically slightly superior to BNP, which is why it has become the most widely used biomarker in the diagnosis and prognosis of patients with heart failure. It is not known with certainty whether NT-proBNP levels are predictors of mortality in patients with this cardiac pathology. Objective: To correlate NT-proBNP concentrations with in-hospital mortality in patients with heart failure admitted to Teodoro Maldonado Carbo Hospital in the 2020-2021 period. Methods: A retrospective, observational, and analytical study was conducted at Teodoro Maldonado Carbo Hospital during the period 2020-2021. NT-proBNP values were obtained at hospital admission for 231 patients diagnosed with Heart Failure. The sample was divided into two groups, the cut-off point being the values ; greater than 3,500 pg/ml and less than 3,500 pg/ml, within the investigation and analysis of results, 108 (46.75%) patients with NT-PROBNP less than 3,500 pg/ml and 123 (53.25%) were obtained patients with values greater than or equal to 3,500 pg/ml and in-hospital mortality was compared. Results: The mortality obtained within the group of patients with values < 3,500 pg/ml was 18 (14.63%) patients, while in the group of patients with values > 3,500 pg/ml it was 73 (67.59%). Regarding the age groups of the patients who died, they were 3 (1.3%) patients, in the second group that includes the ages between 46-65 years, the mortality rate was 17 (7.36%) patients, and Finally, the third group with ages between 66-97 years, mortality was 71 (30.7%) patients, in terms of mortality within the female group it was 38 (16.45) patients, while in the male it was 53 (22.94) patients. Within the group XVIII with the highest mortality suffering from dyslipidemia, it was found that 64 (27.71%) patients with this comorbidity had values greater than 3,500 pg/ml. Regarding Diabetes Mellitus, those who did have this comorbidity, that is, 56 (52.83 %) of them died, and in the cases without Type 2 Diabetes Mellitus, 32 (28%) died. Regarding the NT-PROBNP values of these patients, 63 patients with this comorbidity had values greater than 3,500 pg/ml. Conclusions : NT-proBNP levels obtained at hospital admission predict in-hospital mortality in patients diagnosed with HF, and that this risk of mortality may also be increased by the presentation of the patient's comorbidities already studied.
Descripción : Introducción: El propéptido natriurético cerebral N-terminal (NT-proBNP por sus siglas en inglés) se ha estudiado extensamente por su capacidad predictiva en pacientes con insuficiencia cardíaca . En la actualidad el NT-proBNP ha demostrado ser ligeramente superior clínicamente al BNP, por lo que se ha convertido en el biomarcador más utilizado en el diagnostico y pronostico de los pacientes con insuficiencia cardiaca. No se conoce con certeza si los niveles de NT-proBNP son predictores de mortalidad en los pacientes con esta patologia cardiaca. Objetivo: Correlacionar las concentraciones de NT-proBNP con la mortalidad intrahospitalaria en pacientes con insuficiencia cardiaca ingresados en el Hospital Teodoro Maldonado Carbo en el periodo 2020-2021 Métodos: Se realizó un estudio retrospectivo, observacional y analítico en el Hospital Teodoro Maldonado Carbo durante el periodo 2020-2021. Se obtuvieron los valores de NT-proBNP al ingreso hospitalario de 231 pacientes con diagnóstico de Insuficiencia Cardiaca. Se dividió a la muestra en dos grupos siendo el punto de corte los valores ; mayores a 3.500 pg/ml y menores a 3.500 pg/ml, dentro de la investigación y análisis de resultados se obtuvo 108 ( 46, 75%) pacientes con NT-PROBNP menor a 3.500 pg/ml y 123 ( 53,25 %) pacientes con valores mayores o iguales a 3.500 pg/ml y se comparó la mortalidad intrahospitalaria. Resultados: Se obtuvo que la mortalidad dentro del grupo de pacientes con valores < 3.500 pg/ml fue de 18 (14.63%) pacientes, mientras que en el grupo de pacientes con valores > 3.500 pg/ml fue de 73 (67.59%) , en cuanto a los grupos etarios de los pacientes en estado de defunción fueron de 3 (1.3%) pacientes , en el segundo grupo que comprende las edades entre 46- 65 años , el índice de mortalidad fue de 17 (7.36%) pacientes , y por último el tercer grupo con la edades entre 66- 97 años , la mortalidad fue de 71 ( 30.7%) pacientes, en cuanto a la mortalidad dentro del grupo de sexo femenino fue de 38 (16.45) pacientes, mientras que en el sexo masculino fue de 53 (22.94) pacientes. Dentro del grupo con mayor mortalidad que padecian dislipidemias se encontró que 64 (27.71%) pacientes con esta comorbilidad contaban con valores mayores a 3.500 pg/ml.En cuanto a la Diabetes Mellitus , aquellos que si tenian esta comorbilidad , o sea 56 (52.83%) de ellos fallecieron, y en los casos sin Diabetes Mellitus tipo 2 32 XVI (28%) fallecieron, en cuanto a los valores de NT-PROBNP de estos pacientes, 63 pacientes con esta comorbilidad presentaban valores mayores de 3.500 pg/ml.Conclusiones: Los niveles de NT-proBNP obtenidos al ingreso hospitalario predicen la mortalidad intrahospitalaria en los pacientes con diagnóstico de IC, y que ademas este riesgo de mortalidad puede verse aumentada por la presentacion de las comorbilidades del paciente ya estudiadas.
