Titre: Resistencia antimicrobiana en klebsiella pneumoniae en el Hospital Teodoro Maldonado Carbo, incidencia, tipos de infecciones registradas y tratamiento recibido en período de enero del 2016 a diciembre del 2016.

Résumé: OBJECTIVE: To record the types of Klebsiella pneumoniae infections in patients treated at the HTMC between October 2016 and December 2016 and determine whether or not antimicrobial resistance exists. METHODOLOGY: Observactional, descriptive, retrospective and cross-sectional study, not experimental. The sample size consisted of 149 patients without distinction of sex or age, with cultures positive for Klebsiella pneumoniae infection. RESULTS: 72% of the patients were male, the age range was from 0 to 97 years with an average of 63.3 years. And the average hospital stay was 42.8 days with a range of 0 to 178 days. The mortality rate in the patients studied was 63%. The service of origin where the patients attended when the sample was taken with the highest percentage of patients contributed were the areas of hospitalization of the hospital: 70 (47%), followed by the intensive care area: 63 (42%). Among the underlying diseases prior to taking the sample for culture, hypertension was the most frequent with 88 cases (59%). The most frequent type of infection that motivated the study was pneumonia with 68 cases (46%). The site from which the most common sample was obtained was by tracheal aspirate: 51 (34%). The use of previous antibiotic was evidenced in 117 cases (79%). The drug that showed the highest sensitivity was colistin with 88% and ampicillin with the highest resistance, with 97% 94% producing KPC. Finally, the most used antibiotic treatment was the combination colistin + meropenem (11%) followed by meropenem in monotherapy (8%).
Description: OBJETIVO: Registrar los tipos de infecciones por Klebsiella pneumoniae en pacientes atendidos en el HTMC entre octubre del 2016 a Diciembre 2016 y determinar si existe o no resistencia antimicrobiano. METODOLOGIA: Estudio observacional, descriptivo, retrospectivo y de corte transversal, no experimental. El tamaño de la muestra estuvo constituida por 149 pacientes sin distinción de sexo o edad, con cultivos positivos para infección por Klebsiella pneumoniae. RESULTADOS: El 72% de los pacientes fueron de sexo masculino, el rango de edad fue desde los 0 a los 97 años con una media de 63.3 años. Y la estancia hospitalaria promedio fue de 42.8 días con un rango de 0 a 178 días. La tasa de mortalidad en los pacientes estudiados fue del 63%. El servicio de origen donde los pacientes cursaban cuando fue tomada la muestra con mayor porcentaje de pacientes aportados fueron las áreas de hospitalización del hospital: 70 (47%), seguido por el área de cuidados intensivos: 63 (42%). Entre las enfermedades subyacentes previo a la toma de la muestra para el cultivo, la hipertensión arterial fue la más frecuente con 88 casos (59%). El tipo de infección más frecuente que motivó el estudio fue la neumonía con 68 casos (46%). El sitio del que se obtuvo la muestra más común fue por aspirado traqueal: 51 (34%). El uso de antibiótico previo se evidencio en 117 casos (79%). El fármaco que mostró mayor sensibilidad fue colistina con 88% y ampicilina el de mayor resistencia con un 97% 94% fueron productoras de KPC. Por último el tratamiento antibiótico más utilizado fue la combinación colistina + meropenem (11%) seguido por meropenem en monoterapia (8%). CONCLUSION: La taza de resistencia encontrada en el HTMC fue muy alta para la mayoría de los antibióticos. Se reportó bacterias productoras de KPC en un 94% de los casos. Colistina fue el antibiótico con mejor perfil de sensibilidad, y también el más usado en tramatamiento combinado junto a meropenem. Aunque solo represento el 11% de los casos.
