Abstract: In patients with severe post-COVID-19 Pediatric Multisystem Inflammatory Syndrome (PIMS) admitted to the PICU, it is important to predict the severity of the condition. The PIM III is highly sensitive and specific, and the neutrophillymphocyte (NLR) and platelet-lymphocyte (PLR) indices are simpler, also sensitive, and specific and are calculated on admission with the first blood count obtained. Objective: To determine the association of the neutrophillymphocyte (NLR), platelet-lymphocyte (PLR) indices with the PIM III measured as predictors of severity in PIMS post COVID 19, in children hospitalized in the PICU of the Roberto Gilbert Elizalde Pediatric Hospital, among July 2020 to July 2021. Method: Descriptive, retrospective, crosssectional study, it is descriptive with association of variables and data obtained in medical records. This study included 112 patients between 1 month and 17 years. Absolute frequencies and percentages were used for the analysis of the qualitative variables and for the quantitative variables, dispersion measures were used, ROC curve, sensitivity, specificity, PPV and NPV were calculated. Results: 51.8% of the admitted children were male, 33% were between 1 and 5 years old, the NLR index registered a mean of 5.33, the PLR 336.53 and the PIM III 44 directly related to the severity of the condition. The area under the ROC curve was 0.788 for PLR, 0.750 for PIM III, and 0.683 for NLR. Sensitivity and specificity were 75% and 51.9% for NLR, 83.3% and 42.3% for PLR, and 86.7% and 42.3% for PIM III. All indices presented a significant association with severity. Conclusion: The NLR and PLR indices are simple and useful tools to predict severity in post-COVID-19 PIMS patients admitted to the PICU.
Description: En pacientes con Síndrome Inflamatorio Multisistémico Pediátrico (PIMS) post COVID-19 graves ingresados a UCIP, es importante predecir la severidad del cuadro. El PIM III es altamente sensible y especifico y los índices neutrófilolinfocito (NLR) y plaqueta-linfocitos (PLR) son más sencillos, también sensibles y específicos y se calculan al ingreso con la primera Hematimetría conseguida. Objetivo: Determinar la asociación de los Índices neutrófilolinfocito (NLR), plaqueta-linfocitos (PLR) con el PIM III medido como predictores de severidad en PIMS post COVID 19, en niños hospitalizados en la UCIP del Hospital Pediátrico Roberto Gilbert Elizalde, entre julio 2020 a julio 2021. Método: Estudio descriptivo, retrospectivo, transversal, es descriptivo con asociación de variables y datos obtenidos en las historias clínicas. En este estudio se incluyeron 112 pacientes entre 1 mes a 17 años. Se emplearon frecuencias absolutas y porcentajes para el análisis de las variables cualitativas y para las cuantitativas se usó medidas de dispersión, se calculó curva ROC, sensibilidad, especificidad, VPP y VPN. Resultados: El 51,8% de los niños ingresados eran masculinos, 33% tenían entre 1 a 5 años, el índice NLR registró una media de 5,33, el PLR 336,53 y el PIM III 44 relacionadas directamente con severidad del cuadro. El área bajo la curva ROC fue 0,788 para PLR, 0,750 PIM III y 0,683 el NLR. La sensibilidad y especificidad fue de 75% y 51,9% para NLR, 83,3% y 42,3% para PLR y 86,7% y 42,3% para PIM III. Todos los índices presentaron asociación significativa con la severidad. Conclusión: Los índices NLR y PLR son herramientas sencillas y útiles para predecir severidad en pacientes con PIMS post COVID-19 ingresados en UCIP.
